The following information is required on the application:
One photo of yourself: This photo will be used for the Miss Photogenic and Miss Beautiful Smile competition.
A paragraph about yourself/the applicant.
Proof of residency: For a list of acceptable forms of proof of residency please click here.
Proof of age: Please provide a copy of your birth certificate, license, or passport.

Contestant fee:
$100.00 nonrefundable application fee due when application is submitted on or before March 31, 2025
$200.00 nonrefundable application fee due when application is submitted on or before April 15, 2025
$300.00 nonrefundable application fee due when application is submitted after June 15, 2025
Last date to submit the application is June 15, 2025
